(Mediator) Keeps original copy of Client Grievance Form (Form 2622) or written statement in a special file.

(Mediator) Sends copy of Form 2622 to employee against whom the grievance was made.

(Mediator) Registers all grievances in a log. The log should contain:

the name of the grievant;

the name of the worker(s) against whom the grievance is directed;

the name of the person who conducted the grievance meeting (if it was held);

the issue(s) involved; and

the resolution of the grievance and any appropriate effective dates.
